    Covid Care is a support service that provides clinical care and consults to network patients by referral of their GP. NB: patient or self-referrals will not be accepted, please contact your GP to instigate referral.

    Clinicians can complete this online form to refer COVID-19-positive patients to the Covid Care telehealth service.

    Once in our care, all patients will receive a Post-Consult Advice Sheet containing useful information about managing their symptoms, after-hours care support numbers and the Covid Care team contact details for non-urgent enquiries throughout their covid-19 care plan.
